About the project
The project will focus on interventions and models for a secured patient medication list in care transitions, and for improving patient adherence to evidence based medications.
After a pilot study two separate randomized controlled studies will be performed in cooperation with Region Skåne, Lund University, Kronans apotek, Apotekarsocieteten and Medicpen AB.
